The four examinations of hypertension include aldosterone, cortisol, renin, and adrenocorticotropic hormone.
Purpose of examination: It is mainly used to exclude the causes of secondary hypertension, such as primary aldosteronism, pheochromocytoma, renal artery stenosis and Cushing's syndrome.
Suitable population for examination: The suitable population is young and middle-aged, patients with moderate or severe hypertension, and patients with refractory hypertension.
Aldosterone: is a steroid hormone that is part of the renin-angiotensin system. Mainly acts on the kidneys, promotes the reabsorption of ions and water molecules by the kidneys, and the adrenal cortex secretes excessive aldosterone in primary aldosteronism. The main clinical manifestations are hypertension with hypokalemia.
Cortisol: A type of glucocorticoid. Hypercortisolism is caused by bilateral adrenal hyperplasia due to endocrine disorders, or excessive secretion of cortisol by adrenal tumors. Increased cortisol can cause blood pressure to rise. Cushing's syndrome is a hypercortisol disorder.
Renin: is a proteolytic enzyme that is part of the renin-angiotensin system. It can catalyze the conversion of angiotensinogen into angiotensin and increase blood pressure.
Adrenocorticotropic hormone: Adrenocorticotropic hormone can promote the tissue proliferation of the adrenal cortex and the production and secretion of corticosteroids, and promote the increase of blood pressure.
Inspection precautions: The blood obtained by the test is about 1.8ml of plasma.
The reference values of the four items of hypertension in normal standing are: aldosterone 40-310pg/mL, cortisol 4.26-24ug/dl, renin 14-38pg/mL, and adrenocortical cortex 7.2-63.4pg/mL.
